Kehjistan might be the last region of Sanctuary that players get around to exploring in Diablo 4. But not necessarily, thanks to the game's awesome open-world design. You can travel to this harsh desert land through either the Dry Steppes by Jirandai, or Hawezar where the two regions meet at the southernmost edge of the map.

While Strongholds typically vary in difficulty, the Kehjistan Strongholds are brutal. Definitely bring a team, and make sure your gear and potions are solid for your level range. Alcarnus

The first and most obvious of Kehjistan's Strongholds, Alcarnus can be found directly south of Jirandai and north-west of Tarsarak. Traveling south from Alcarnus and hugging the western cliff until you find a narrow canyon is the easiest way to get there.

Once a peaceful town, Alcarnus is now under the curse of a powerful witch. You'll have to defeat her many minions and deal with her extremely powerful magic attacks to conquer this Stronghold. Unfortunately, doing so doesn't unlock any new dungeons or Waypoints. But there is an Altar of Lilith within Alcarnus, and another to the west.

Altar Of Ruin

Getting to this Stronghold from the Iron Wolves Encampment takes you through some winding canyons to the northeast. If traveling from Alcarnus, continue through the canyon to the west and then traverse the desert northwest to reach the Altar of Ruin.

Like other dark places of worship in Diablo 4, the Altar of Ruin is full of evil cultists. But, this is easily the most rewarding Stronghold in Kehjistan. There's an Altar of Lilith here, as well as a Waypoint and a new dungeon. Necromancers can find the Hulking Aspect there. The Baal statue is also at the Altar of Ruin, which is required for The Way of the Three side quest.

Omath's Redoubt

This Stronghold can actually be pretty difficult to find unless you know where to look — although if you enjoy PVP, it may be easier, since it's just southwest of Kehjistan's Fields of Hatred. You can also get there very quickly from Gea Kul by the southernmost canyon.

Be prepared to face many undead hordes at Omath's Redoubt, along with a necromancer for their Boss. You can find an Altar of Lilith in Omath's Redoubt, and two more on the way if you travel from Gea Kul. Clearing this Stronghold will give you access to a new dungeon with the Sorcerer's Aspect of Splintering Energy. Although there's no new Waypoint to unlock, the one northeast in the Fields of Hate is already very close.
