As Diablo 4 reaches its climax players enter Hell - racing Lilith to Mephisto's realm before she can absorb his essence. During this time, the difficulty of fights ramps up, and bosses come one after another.
One such boss is Ninsa, a succubus loyal to Lilith. She has been sent to delay your character, allowing Lilith enough time to accomplish her goal. As a powerful demon, Ninsa doesn't go down easy. You will have to survive several onslaughts to be victorious. Keep reading for everything you need to know about fighting Ninsa, Blight of Hatred.
Preparing For The Fight
To prepare for Ninsa, use the town portal just after speaking with Prava in the Burning Hells. This will be one of the last lulls before heading into nonstop action.
Once in town, repair your gear, upgrade it, and stock up on Elixirs of Demon Slaying. These will help your damage against the predominant enemy type and boost your experience gain. This section takes most players 2 - 3 hours to clear, meaning you'll want about six elixirs.
All Ninsa Fight Locations
The first time the player encounters Ninsa is during the quest In Desolation's Wake. She will initially appear on the road in the Hells (yellow circle), before moving into a building (red circle).
Fighting Ninsa, Blight of Hatred
Ninsa is a special boss among the few to spawn with more than three phases. Additionally, fighting her takes place over two encounters with a different boss in the middle.
Phases & Stagger Bar
Ninsa has four phases instead of three in her first appearance - this is misleading, as she will flee when at 75% health. During her second appearance in the Searing Expanse, she only has three phases.
Like all bosses in Diablo 4, Ninsa has a stagger bar. Being staggered does not prevent her from fleeing, however, which will happen regardless of any status conditions when triggered.
Ninsa does not gain significantly different attacks between phases or appearances, but the size and speed of her attacks get bigger. Luckily, during the toughest part of the fight, you will be able to count on your allies to soak attacks and do extra damage.
Damage Types
Ninsa does primarily fire damage, courtesy of her large AoE attacks. However, she also does some lightning damage with her small projectiles.
The Hound of Hell
To cover her initial retreat, Ninsa sends Karum, Hound of Hatred after you and your companions. This is a multiphase boss encounter in its own right, but Karum is easily defeated. It seems his existence from a game design perspective is to drain more of your resources before fighting Ninsa again.

Ninsa has only two attacks of note - most of her threat comes from minions.

Sidestep these balls whenever possible. If necessary, you can use health potions to heal through them. |
In addition to her own attacks, Ninsa will summon a vast variety of minions. During the first fight:
- She will spawn invulnerable, protected by Soul Burner minions. Kill these to break her shield.
- Endless waves of Spawns of the Damned will charge you while you whittle her down - clearing these with AoE is important to avoid getting overwhelmed.
In her second appearance, Ninsa summons stronger demons to fight at her side. Players will need to fend off:
- Sin Eaters
- Balrogs
- Spawns of the Damned
- The Annihilator
Again, AoE attacks are crucial to clear these minions while still damaging Ninsa.
Rewards
Ninsa drops three rare items and a little bit of gold when defeated. Killing her also allows you to continue with the story. However, there are no special rewards, and she does not appear to drop Legendary Gear.
